Comprehensive health services provided in a patient-centered medical home. Primary medical care plus integrated behavioral healthcare. High-quality care provided by physicians, nurse practitioners, physician assistants, psychologists, and licensed certified social workers. 

We offer all patients care they can afford. We see patients covered by private insurance (Excellus BCBS; Guardian; United Concordia; UnitedHealthcare; MVP; and Fidelis to name a few), Medicaid and Medicaid Managed Care, Military Insurances, Medicare, Child Health Plus, and we offer a sliding fee scale for patients with no insurance. We have collaborative agreements with both Samaritan Health and Lewis County General hospitals to support our medical care. If you should have any concerns about your healthcare coverage please contact our main office at, 315-782-9450.

Comprehensive dental care provided to patients of all ages. Appointments for children as young as 6 months. High-quality care provided by dentists and dental hygienists who are expert at putting children and adults at ease.

    Everyone is welcome at North Country Family Health Center:
    We accept private insurance, military insurances, Medicaid, Medicare, Family Health Plus and offer a reduced fee to patients without insurance and those who are unable to afford/pay their high deductible or co-pay.

  • Accessible

    24-hour on call service:

    When our office is closed, we are still available to our patients.

    We welcome patients with disabilities, have translation and interpretation services available for patients who do not speak English and are always happy to help patients who have trouble reading, writing, or understanding medical information or forms.

  • Comprehensive

    We offer more than medical care on-site.

    If we don’t offer the care you need, our health care team works with you to coordinate your care both inside and outside of our office.

Jana Shaw, MD, Chief Medical Officer

Jana Shaw, MD, MPH is a practicing pediatrician specializing in infectious diseases and an expert on vaccines and immunology. Dr. Shaw is Associate Professor of Pediatrics at SUNY Upstate Medical University, Syracuse, New York and is chief medical officer at North Country Family Health Center. She is a member of pediatric and infectious disease professional societies and a member of the International Affairs and Communications Committees of the Pediatric Infectious Disease Society. Her research interests focus on improving vaccination coverage for children. She has authored numerous publications focusing on vaccine hesitancy and is an active vaccination advocate.

Scott Stern, MD

Dr. Stern is a family practitioner who provides care for both adults and children. He is certified by the American Board of Family Practice.

Dr. Stern completed his undergraduate education at the State University of New York at Binghamton and attended the State University of New York Health Science Center in Syracuse, where he received his M.D. degree in 1997. Dr. Stern completed a family practice residency at Guthrie Family Practice in Sayre, Pennsylvania where he served as chief resident from 1999-2000.

Before Joining North Country Family Health Center Dr. Stern was an attending physician at Parish Health Center in Parish, NY from 2003-2015 and with Lowville Medical Associates in Lowville.
