The Care Coordinator works as part of a team with Patient Service Representatives, Providers, Family Practice Administrator, RN Clinical Care Coordinator/Chronic Care Manager, and other Care Coordinators. Health Informaticist and Billing, to ensure smooth continuum of care for identified patients, many of which are high risk. This includes customer service, making follow-up appointments, referrals/referral follow up and documentation, telephone calls to patients, ensuring patients are able to make scheduled appointments and employing enabling services as needed, directing inquiries, filing and assisting medical team.
Certification as a Medical Assistant or Nursing Assistant required. Administrative skills to include knowledge of patient care, medical terminology, access, and compliance issues. Has excellent interpersonal and verbal communication skills. Is skilled with computers and computer applications. Gives attention to detail and possesses problem resolution skills. Has a positive attitude and high energy, is a quick learner, is adaptable to change and is resourceful and proactive when identifying and resolving problems.
